Only babies born in the Town of Fort Erie must be registered at the Town hall in Fort Erie. This process ensures that the baby will have a legal identity such as a birth certificate, passport and other identity documents.
New on-line services : You can now complete forms for your child's birth registration, birth certificate and Social Insurance Number on line through the Province of Ontario. To begin, go to www.serviceontario.ca and click "Life Events". Please remember, if you use the on-line birth registration form, you must print a copy and send it with payment ($20.00) to the Town of Fort Erie.
Once you have submitted the forms to the Town, you don't have to do anything else until you get a letter from the Province of Ontario confirming you child has been registered.
Here's what happens to your forms:
- The Town Clerk checks the form and matches it with a notice that the Midwife sends to the Town. If everything is complete, the Town of Fort Erie will process you payment, register your child and forward the documents to the provincial Office of the Registrar General.
- The Office of the Registrar General will register your child and once complete, you will receive a letter from the provincial Registrar General stating that your child has been registered. (This process can take six months or longer, from the time all the forms are received.
Once you have received the acknowledgment from the Office of the Registrar General, you can obtain a birth certificate on line at www.serviceontario.ca. You can also contact the office of the Registrar General directly at 1-800-461-2156 or visit their website: www.mgs.gov.on.ca
